DIAGNOSIS This is an HDMI handshake failure, where the TV and console don't agree on a video signal format.
FIX Power cycle the entire chain in order. Turn off the PlayStation. Unplug the TV and the PlayStation from power for 2 minutes. Plug the TV in, turn it on, then plug the PlayStation in and turn it on. Use the TV remote to manually switch to the correct HDMI input.
IF THAT DIDN'T WORK Change the PlayStation's video output resolution manually. Boot the PlayStation in safe mode (hold power button for 7 seconds until second beep), then select 'Change Resolution'. Set it to 1080p, then see if the picture returns, then adjust back to 4K if needed.
HEADS UP This TV's HDMI ports can be finicky with high-bandwidth signals. Ensure you're using a Premium High-Speed HDMI cable and try a different port (HDMI 2 or 3).
